Support Co-ordination
Our experienced support coordinators will assist participants at every step while they look forward to implementing their NDIS plan along with identifying the supports and services that they need. We are the most efficient NDIS service providers for your requirements and can help you devise a plan as per your individual need.
Our support coordinators assist participants to increase their capacity to maintain (or in some cases change) support relationships, resolve service delivery issues, and participate independently in NDIA processes. Our services also aim at providing participants with the assistance they need to get started with their NDIS support plan and connect them with the best providers that align with their needs.
At LWCC our support coordinators understand participants’ requirements, their current NDIS plan, and assist participants to utilise their NDIS plan as effectively as possible, and answer questions as they arise for maximising their support.
